Injury & Performance Chiropractic operates this website. This policy explains what happens to information you submit here. It is separate from the HIPAA Notice of Privacy Practices that governs your protected health information as a patient of the practice — that notice is provided to you at the clinic.
Information We Collect
We collect information in two ways:
- Information you provide. If you submit the contact form, we receive the name, phone number, email address, reason for contact, preferred contact method and message you enter.
- Standard technical information. Our hosting provider records routine server data such as IP address, browser type and pages requested, as part of operating and securing the site.

The contact form is a convenience for scheduling and general questions. It is not a secure or encrypted channel for protected health information. Please keep messages general and save clinical detail for your appointment or a phone call to (813) 553-2269.

We do not sell your personal information, and we do not share it with third parties for their own marketing purposes.
Service Providers
This site relies on a small number of third-party providers, each of which may process limited data in the course of delivering their service:
- Netlify — website hosting and contact-form processing. Form submissions are stored in the practice's Netlify account.
- Google Maps — the embedded map on the Contact page is served by Google and is subject to Google's own privacy terms.
- CharmHealth — the "Book Now" button opens the practice's online scheduling page, which is hosted by CharmHealth. Any information you enter there is governed by that platform and by the practice's patient privacy practices.
Cookies & Analytics
This website does not set advertising or tracking cookies of its own, and no analytics package is installed. Embedded third-party content such as the Google Maps frame may set its own cookies when it loads.
Data Retention
Contact-form submissions are retained only as long as needed to respond to your inquiry and to maintain a reasonable business record, after which they are deleted.
Your Choices
You can ask us what information we hold from a website submission, ask that it be corrected, or ask that it be deleted. Contact us using the details below. Rights relating to your medical record as a patient are handled through the practice's HIPAA Notice of Privacy Practices rather than this policy.
